Information Page for Sonoma County CHWs/Promotoras de Salud/Representatives
Community Health Workers/Promotores de Salud/Community Health Representatives are essential members of the healthcare and public health workforce. They have numerous titles, including but not limited to peer support specialist, lay health worker, and patient navigator and can be found in community based organization (CBO), health center, school, and government spaces. These individuals have unique lived experiences and knowledge, and foster trusting relationships. They serve as intermediaries and the bridge between health/social services and their communities to facilitate access to services and improve the quality of service delivery. Similarly, they educate providers, organizations, and health systems about individual and cultural expectations.
