Disease Reporting

The state of California requires doctors, surgeons, nurses, practitioners, coroners, dentists, veterinarians, administrators of health facilities and clinics to report cases or suspected cases of certain diseases and conditions to the local health department. Anyone in charge of public or private schools or preschools also must report cases or suspected cases of these diseases.

How to Report

The Confidential Morbidity Report (CMR) forms listed below are used to report most reportable diseases and conditions. 

To report all conditions except those listed below: CMR CDPH 110A

To report Tuberculosis (TB): See Tuberculosis Control for more information

To report conditions which may impair the ability to operate a motor vehicle, such as lapses of consciousness or control, Alzheimer's disease: CMR CDPH 110C

To report HIV/AIDS: See HIV/AIDS Reporting for more information
